**BAYADA Home Health Care** is currently seeking an experienced **Weekend Per Diem** **Physical Therapist** to provide exceptional care to **geriatric patients** in **Independent Living and Assisted Living Communities** in **Canton, Millis, Dedham and surrounding towns.** Prior home health experience is required. As a home health physical therapist you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, affording them the opportunity to receive the medical care required to maintain their independence.
