**Job Summary and Responsibilities**

As our **Virtually Integrated Care RN** , you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You'll help bridge care gaps and enhance engagement in a dynamic virtual Medical/Surgical/Telemetry environment.

Every day you will meticulously assess patient needs, develop personalized care plans, and provide direct clinical coaching. You will be expected to proactively monitor progress, coordinate diverse services, and leverage telehealth, diligently tracking compliance, documenting interactions, and contributing to quality improvement.
