Job Description

The Vascular Acute Care Advanced Practice Nurse (APN) or Physician Assistant utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.  
The APN will be primarily responsible for IP rounding and consults.  Raritan Bay Medical Center -Old Bridge, Perth Amboy, JFK (Edison) 
Schedule will be Mon- Fri 8-5 - on call 1 weekend a month 
Wound Care cert preferred. 
Must have an acute care certification as a Nurse Practitioner.
