Transition Support Worker

We are seeking an individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, and it will run for 12 months initially, but the hope is that this role will become permanent, so we can continue to support young people from our CAMHS inpatient settings to the next stage in their recovery.

As a team we adopt a holistic approach, not only focusing on mental health but also addressing the physical, social, educational, and motivational needs of the young people we work with. By employing the social prescribing model, we help young people engage with life through activities that promote community connections, sports, education, employment, housing, and more. Your role will empower young people to build confidence, improve wellbeing, and work towards their long‑term health goals.
