Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
- Educate patients and family members on home exercise programs and safe functional mobility
- Active Physical Therapist (PT) license in the state of Maryland
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a SNF or similar post-acute care environment
