Responsibilities:
- Perform home health therapy visits for adult patients, including start-of-care and follow-up assessments.
- Document patient progress accurately within required timeframes and update EMR systems such as HCHB and PointCare.
- Travel up to 75 miles from the home clinic to patient locations, managing a caseload of 5-6 patients per day.
- Participate in periodic office meetings and collaborate with the healthcare team to deliver comprehensive patient care.
