Provides skilled rehabilitative care to patients with physical functioning disorders under the supervision of a Physical Therapist, utilizing evidence-based practices to support treatment plans and optimize patient outcomes.
Responsibilities:
- Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, and use of supportive devices.
- Assess patient progress and communicate with supervising Physical Therapist to modify treatments accordingly.
- Document all clinical activities, patient progress, and regulatory compliance in the electronic medical record system.
- Provide education, counseling, and support to patients, families, and caregivers regarding physical health and treatment goals.
- Collaborate with the healthcare team to develop discharge plans and treatment strategies.
