Provides clinical evaluation, treatment, and rehabilitation services to patients with physical functioning disorders and wound care needs. Develops and implements individualized care plans following regulatory and clinical standards to improve patient outcomes.
Responsibilities:
- Screen, assess, and evaluate patients to determine their physical therapy needs and develop appropriate treatment plans.
- Administer manual techniques, therapeutic exercises, modalities, wound care, and use of supportive devices as part of the rehabilitation process.
- Monitor patient progress, document clinical interventions, and modify treatments based on ongoing assessments and outcomes.
- Provide education, counseling, and support to patients, families, caregivers, and healthcare team members regarding treatment plans and progress.
