Supportive Housing Case Manager

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, theSupportive Housing Case Managerwill provide case management services to residents of our brand-new Supportive Housing program,Confluence Court, in Binghamton, NY. The Case Manager will provide services for up to 30 residents receiving ongoing care for Substance Use Disorder with the support of a team of their Team Leader and Peer Specialists.The program's planned opening is August 2025, but we are looking to hire a Team Leader immediately to begin the process of promoting the program and accepting referrals. The position initially starts via remote work but will transition to an on-site role upon the opening of the program.
Pay Rate:$25.00 - $29.00 per hour
