About the Role
The Transition to Adulthood (T2A) team at Churches of Christ supports young people as they move from out of home care into independent adulthood. Funded by the Department of Child Safety, Seniors and Disability Services (DCSSDS) through the Extended Post Care Support and Next Step Plus initiatives, our programs are evidence informed, structured, and tailored to the unique needs of each young person.
Our practitioners work alongside young people and key stakeholders to build essential life skills, strengthen support network...
