**SUMMARY OF POSITION:**

Provide medical services and contribute to the design and delivery of high quality, cost effective, values- based patient care for our patients.

**MINIMUM QUALIFICATION(S):**

+ Medical degree from an accredited medical school
+ MD, DO, or credentialed licensed provider
+ Must be board certified and currently possess a license to practice in State of employment
