Job Description
A large home healthcare organization is seeking a Project Manager to join its Portfolio and Transformation Management group. This individual will support the finalization of the Statement of Work (SOW) and onboarding process for the implementation of a new EMR system within the company’s Hospice practice area. The role will involve assisting with SOW negotiations and finalization, followed by leading the EMR implementation, from inception through deployment.

The Project Manager will work closely with the Hospice practice area to ensure alignment, accuracy, and stakeholder satisfaction, while holding the EMR vendor accountable for agreed-upon timelines and deliverables. In addition, this role will partner with internal IT teams to ensure proper data migration and system integrations, all while maintaining strong change management practices throughout the implementation.
