Opportunity to join the Urgent Community Response Team, based with the Community Integrated Response Team. The role will work across Frailty Virtual Wards, Urgent Community Response and Home First. The service operates extended hours 7 days per week providing cover 8 am–8 pm. You will be expected to cover service demands across the extended hours and weekends on a roster basis.

Main duties of the job

Enthusiastic and forward‑thinking Specialist Occupational Therapist to join a busy, pro‑active, integrated service based across St. Catherine's Hospital and Arrowe Park Hospital.

Responsibilities: complete comprehensive assessments for patients with a wide variety of physical and mental health conditions and complex social issues; implement treatment plans and collaborate with MDTs. Liaise and refer into wider health, social care and voluntary agencies when appropria...
