Job Description
Main duties of the job Clinical and professional practice- Provide a specialist clinical pharmacy service in the community setting, ensuring safe, effective and evidence-based use of medicines. Conduct comprehensive medication reviews, support high risk medicines optimisation, and promote adherence while working collaboratively within multidisciplinary teams.
Safety- Reduce medication-related risks by supporting safe practice through identifying, reporting and investigating incidents.
Leadership- plan and manage own workload, support pharmacy service modernisation, and contribute to the development of medicines optimisation initiatives in the community.
Education and training- Provide training and education that meet statutory, professional and regulatory requirements for pharmacists and technicians.
