Social Worker (Yishun)

• Engage identified students regularly to provide mentoring and emotional support
• Build positive relationships with students and contribute to their holistic development
• Plan and conduct student‑centric interventions (outreach, home visits, family sessions)
• Facilitate group‑based programmes for students with social, emotional, or behavioural needs
• Identify student concerns and escalate cases to school leaders and support teams
• Participate in case review meetings with counsellors, SEN officers, and school personnel
• Support referrals to external agencies and community services when required
• Maintain accurate case notes, documentation, and progress updates
