About the Role

To provide social work services to patients and their whanau of Te Whatu Ora- Southern by:

• Undertaking social worker assessments, interventions and evaluations
• Working with the MDT to ensure safe care plans
• Protecting vulnerable patients from harm because of abuse and neglect
• Collaborating community service providers to deliver wrap‑around supports for patients
