Social Worker - Part Time, Emergency Department

Job Summary:
In this position, the Social Worker in the Emergency Department will provide comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. Knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advance directives. In addition the social worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual's health needs.
