What you will do in this role:

+ Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support
+ Research and advocate for appropriate public assistance resources for clients
+ Referral to community resources, assistance with socio-economic problems
+ Communicate with client’s care team
+ Provide crisis intervention (referral to DCF, provider, etc as needed)
+ Coordinate treatment planning with specialists and outpatient providers...
