Main duties of the job

As Social Worker within the Older Peoples Community Team, you’ll assess the needs of people aged 65+ and develop personalised care plans that promote independence, wellbeing and quality of life. You’ll work closely with individuals, families and carers, using strength‑based approaches to support people to stay connected to their communities.
