We are looking for a full time social worker to join our Gedling Local Mental Health Team (LMHTs).

Job responsibilities

• Maintain an individual caseload and complete team responsibilities.
• Assess and analyze risk and ongoing risks to develop personalized risk and safety plans, balancing risk management, safeguarding, choice and self‑determination.
• Provide psychoeducation interventions and social work support to individuals experiencing social issues that impact their mental health.
• Work within a multidisciplinary team to improve mental health outcomes through understanding the social determinants of health.
• Represent the social work and social perspective in the team and lead on the trauma‑informed social model of mental health and wellbeing.
• Deliver evidence‑based practice informed by social work interventions, theories and values.
