Adults Hospital Discharge Social Worker (Locum)

£38 per hour, 6‑month initial contract.

Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Aexander Hospital, Portsmouth.

Role will involve triangulating, resolving issues in a fast‑paced environment, safeguarding, managing expectations, speaking with patients and their next of kin, and managing a task list.

Responsibilities

• Work with the Advanced Assessment Team (AAT).
• Be part of a multidisciplinary sub team supporting early intervention in the hospital to reduce length of stay.
• Complete Care Act assessments and Mental Capacity Act assessments as needed.
• Build professional MDT networks with key areas of the hospital.
• Conduct risk assessments and liaise closely with community teams around high intensity users to turn these individuals around.
