**HARBORVIEW MEDICAL CENTER SOCIAL WORK** has an outstanding opportunity available for a **SOCIAL WORKER - INPATIENT CARE MANAGEMENT BED READINESS PROGRAM** .

**WORK SCHEDULE**
• Per Diem
• Day Shift

**POSITION HIGHLIGHTS**
• This position will provide care coordination for an effective early discharge process.
• This is a two-year funded State Project, focusing on Medicaid Fee-for-Service, Managed Medicaid, and Dual Medicaid-Medicare eligible adult clients who are medically ready to be transferred outside of an acute care setting but are unable to due to transition complexities and barriers.
• The Social Worker, in conjunction with the patient and interdisciplinary care team, identifies the clinical and financial targets and milestones for discharge and helps to eliminate barriers as possible.
