Location: Bohemia, NY | Schedule: Full time 35/hours | Schedule: Mon-Fri evening and weekend hours, as needed | Status: Non-Exempt | Rate: $22/hour

The Opportunity

Provide education and develop independent living skills in foster care children referred by the Department of Social Services in accordance with agency and regulatory requirements to ensure the highest quality of client care.

Responsibilities


+ Conduct monthly home visits.

+ Assist in developing and facilitating Independent Living Skills Workshops

+ Complete a Step Up Independent Living assessment and treatment plan in partnership with client, foster family and caseworker.

+ Complete and maintain client case records in a timely manner and in compliance with agency and other regulatory standards.
