Social Worker

A nursing home in Nashville is seeking a Social Worker to join their team.

Social Worker Job Description:


+ Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs

+ Identifying individual social and emotional needs

+ Developing and maintaining individualized care plans

+ Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

+ Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

+ Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay

+ Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
