Social Prescribing Link Worker

This is a dynamic, community-focused position within a centralised model of care. You'll design and deliver monthly themed health initiatives, offer 1:1 support, and lead group sessions aimed at improving wellbeing among groups such as older adults or frequent attenders. Your work will directly address social determinants of health, connecting people to non-clinical support and empowering them to thrive.

You will work in partnership with the 7 GP practices and other care providers that are based in the central and north Derbyshire Dales from Matlock in the centre up to and including Hope village on the South Yorkshire Border.
