The Mental Health South team in Worcestershire are a team focused on supporting and empowering individuals to have the best mental well-being possible, to reach their goals, and to maximise the quality of life of those we support.We work closely with our colleagues in health, commissioning, safeguarding and other partner agencies to create creative care plans, manage risk, and support discharge plans from out of county and rehab mental health wards.We support individuals from 18+ with functional mental health conditions as well as co-morbidities and support the transition process from children to adult’s services when required.

We assess individual needs based on the Care Act Eligibility and when applicable assess needs to ensure we are meeting our S117 aftercare responsibilities under the Mental Health Act. We also oper...
