Senior Social Worker - Primary Aligned Care Team (PACT)

Summary The primary Aligned Care Team (PACT) Social Worker Lead in conjunction with the PACT Social Work Supervisor, is responsible for clinical program oversight, program effectiveness and service pattern modification for social work services provided within the Primary Care Service. The incumbent assures high quality, professional clinical social services are provided to Veterans, their families and caregivers in the Primary Care clinics at the VA Medical Center, satellite and outpatient Clinics.
