Working Pattern: 36 or 18 Hours per week (preference to be discussed at interview stage)

Responsibilities

• Assess, plan, implement, monitor and review the individual care plans using National Care Standards, establishing and maintaining sound administrative systems including service user records, medication management and stock control, rotas, attendance, risk assessment and compliance with health and safety, data protection and confidentiality policies.
