Senior Nurse

Are you an experienced and compassionate nurse looking to make a real difference to the lives of some of the most vulnerable patients within the community?

We are seeking an enthusiastic and highly skilled senior nurse to join our Integrated Neighbourhood Team, working across a Primary Care Network of three GP practices. This is an exciting opportunity to work collaboratively within a multidisciplinary team delivering high-quality, patient-centred care to frail patients, those requiring palliative support, and individuals approaching end of life.

The successful candidate will play a key role in supporting patients to remain safely within their own homes, preventing unnecessary hospital admissions, and delivering holistic care tailored to individual needs.

Full or part time hours considered.

Pay depending on experience.

Full UK driving licence and use of own vehicle is essential for this role.
