Responsibilities

• Monitor the wellbeing of each resident, ensuring any changes impacting care and care plans are identified and documented.
• Participate in person‑centred care planning and conduct monthly evaluations.
• Support residents with medication administration.
• Conduct staff supervisions and assist with staff development.
• Act as a key point of contact for residents, relatives, visitors and health and social care professionals, addressing reasonable requests and escalating concerns or complaints.
