Primary Responsibilities (including, but not limited to):

• Complete all documentation for nursing assessments, observations, and planning evaluation pertaining to clients
• Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan
• Coordinate with management on implementing and revising operational procedures related to quality client care
• Utilize professional assessment skills to identify physical, mental, and psychosocial needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary...
