The Revisit Nurse ensures quality and safe delivery of nursing and home health services rendered. He/She coordinates provision of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned adult, geriatric, pediatric, and neonatal clients. Ensures quality and safe delivery of nursing care services.
Essential Duties and Responsibilities:
* Responsible for carrying out the plan of care for the adult, geriatric, pediatric, and neonatal client as developed by the nurse case manager while addressing all problems in the plan of care, identifying interdisciplinary care needs and making recommendations to the nurse case manager for additional home health care services.
