**SUMMARY OF POSITION:**

A Registered Nurse Navigator role aims to help facilitate the patient journey to ensure proper navigation throughout the health care system upon discharge disposition. The RN Navigator also aims at supporting staff to assist in the coordination of care while the patient is transitioning through the healthcare continuum throughout the outpatient healthcare setting.
