**Registered Nurse (RN) Case Manager – Hospice**

**Location:** Marlborough, MA and surrounding towns
**Duration:** 13 weeks
**Start Date:** 2–3 weeks from job offer
**Schedule:** Monday–Friday, 8:00 AM – 4:30 PM
**Hours:** 40 hours per week

**Compensation**

+ **Pay Rate:** $63/hour
+ **Mileage Reimbursement:** $0.72 per mile

**Responsibilities**

The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimal utilization of resources, effective service delivery, and compliance with prescribed medical regimens.

Key responsibilities include:
