RN - Continuing Care (Rehab, Long Term)

• Deliver direct patient care to short-term rehab and long-term care residents
• Manage high patient ratios (up to 20+ patients) efficiently and safely
• Perform and monitor trach care, wound care, and dressing changes
• Administer medications and manage IV therapy and tube feedings
• Provide Foley catheter care and monitor patient progress
• Collaborate with therapy teams, case management, and nursing leadership
• Support patient recovery and rehabilitation goals through continuous assessment
• Accurately document patient care using EMR systems (Epic preferred)
• Float between units or nearby facilities as needed

• Active RN license
• Minimum 18 months of recent SNF/LTC/Rehab experience
• Strong experience in long-term care and rehab nursing
• BLS certification (required)
