Looking to make an impact for patients and families in your community? HealthCare Support is seeking a Registered Nurse (RN) Patient Care Manager in Brooklyn Park, MN. In this leadership role, you’ll oversee clinical operations while supporting a team delivering high-quality, personalized care to patients in their homes.
Daily Responsibilities:
Oversee a patient census of ~150–175 patients and manage a team of 10–20 clinicians and support staff (RN, LPN, PT, OT, PTA, COTA, SLP, MSW) 
Provide clinical supervision to ensure patient care aligns with professional standards and regulatory requirements 
Coordinate, plan, and monitor patient care while ensuring schedules and care delivery are efficient and effective
