RN Case Manager PRN

Registered Nurse Home Health PRN Medford

Schedule: Monday - Friday 8:00 a.m.- 5:00 p.m./PRN as needed

Coverage Area: Medford and Surrounding Areas

Responsibilities:

· Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.

· Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).

· Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.

· Offer education and emotional support to patients and their families/caregivers based on individual needs.

· Supervise LPNs/LVNs, home health aides, and other support staff.
