RN Case Manager - Pediatric Clinic

**Regular, Full-Time position in the Talihina Pediatric Clinic | Pay: paygrade 13, Hourly, dependent on experience | Schedule: Mon-Fri 8a-4:30p**

**Job Purpose or Objective(s):** As the Case Manager RN, you will provide case management services and coordinates the delivery of health care for assigned patients. You will supervise other members of the health care team. You will report to the Manager or Director over the department.
**Primary Tasks:**
1. Assess and evaluate patient needs to prepare and implement treatment and case management plans.
2. Confer with health team members to discuss and evaluate patient information for planning, modifying, and coordinating treatment programs.
3. Explain procedures and treatments to patient to gain cooperation, understanding, and allay apprehension.
4. Administer prescribed medications and treatments in accordance with physician orders.
