Full time RN Case Manager in the inpatient setting at Trinity Health Grand Rapids Hospital

**This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health is a Magnet Designated Hospital! This role will provide case management to our patients and families in the acute care setting.**

Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.

+ Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire
