Position: Renal Care Coordination Team Leader

Location: Centurion, Gauteng

Position Purpose: As the Renal Care Coordination Team Leader, you will lead a specialised team of clinicians who provide comprehensive support to beneficiaries with chronic kidney disease (CKD), End‑Stage Kidney Disease (ESKD), and those undergoing dialysis or post‑transplant care. You will play a pivotal role in managing clinical and financial risk, improving patient outcomes, enhancing treatment adherence, and ensuring smooth coordination between nephrologists, dialysis centres, transplant units, and internal stakeholders.

Experience & Qualifications:

Registered Nurse (3- or 4‑year qualification) OR health sciences degree — mandatory

3 to 5 years’ experience in:

Renal nursing or nephrology case management Dialysis (hemodialysis or peritoneal dialysis) pathways Managed care, health risk management, or clinical customer service Previous experience managing or supervis...
