Program Description

The Mobile Mental Health Team delivers non-crisis, short-term care to individuals aged 16+ transitioning between acute episodes and long-term support. Services are primarily delivered in clients’ homes, with flexibility to meet in public spaces or virtually, based on client preference. The team serves Huron and Perth Counties.

Key Responsibilities

• Conduct in-home and community-based client visits
• Provide therapy and counselling using evidence-based practices
• Assess mental health and risk, including suicidal, self‑destructive, homicidal, and aggressive behaviors
