RN Case Manager - Position Highlights  
Location: Salinas, CA  (Hybrid - Remote) 
This is a hybrid, field based role requiring a dedicated home office and the ability to travel 3-4 days per week within an assigned territory. Coverage includes the Salinas, CA market and surrounding areas. Territory assignments may change cased on business needs.  
Schedule : Monday - Friday 
Flexibility is required to accommodate patient needs and nephrology partner availability.
