The exciting opportunity

• Part-time role with a variety of shifts available
• Deliver high quality clinical care to residents of our home in Spearwood, WA. 
• Conduct and analyse assessments to determine care needs
• Formulate and update person centred care plans aligned to residents needs and wishes
• Complete comprehensive documentation and progress notes that accurately reflect the care delivered
• Provide supervision and direction to Personal Care Workers
