Summary The North Florida/South Georgia VA Healthcare System's Social Work Service is seeking an experienced and dynamic Registered Nurse Outpatient Caregiver Support Program Clinical Assessor. Our ideal applicants are those who want to contribute to the achievement of quality care standards, as well as the growth and development of a VA academic hospital program. Preferred experience with 1-2 years case management in a primary care/ mental health setting. Responsibilities The Caregiver Support Program Clinical Assessor Nurse is responsible for knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day to day operation of the program, and all reporting requirements within the assigned Veterans Integrated Systems Network.
