Reporting to the Care Manager, the Case Manager is responsible for directing the care of clients under the Home Care program. Using your strong assessment, organizational, and leadership skills, you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching.
