Position Summary

Registered Nurse with a passion for delivering compassionate patient care to join Chetwynd General Hospital. The Primary Care Nurse (PCN) works under the general supervision of a Manager and follows standards of skilled practice and the BC College of Nurses and Midwives (BCCNM) code of ethics.

Primary Care Networks are partnerships between BC family physicians, regional health authorities, First Nations Health Authority, and community partners, partnering to redesign primary and community care systems for better care delivery.

Responsibilities

• Provide direct clinical care and services to individuals, groups, and communities in settings such as physician practices, homes, schools, and communities.
• Follow clinical follow‑up according to the person’s Primary Care Plan and personal goals of care, informing plan development with the interprofessional team.
