**Position Summary:**

Facilitates and coordinates the process for all acute transfers in to the organization in accordance with the clinical standards and guidelines. Manages all calls related to patient transfers to Trinity Health Grand Rapids (THGR) from acute care facilities, physician offices and long term care seeking to have patient admitted to THGR or evaluated in the Emergency or OB department.

**Minimum qualifications:**

+ Bachelors Degree in Nursing or other health related field is required
+ Current Michigan license as a Registered Nurse
+ Active BLS certification
+ Current knowledge of patient care practices and health care systems
+ Five years of current and progressive clinical and leadership experience in an acute care setting, including involvement in direct patient...
