RN – Care Manager

Schedule: Days, 8 hours per shift, 40 hours per week

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Must possess a valid nursing license.
• Experience in home health care preferred.

Roles & Responsibilities:

• Plan, organize, and direct home care services to meet patient needs.
• Manage a caseload effectively and efficiently.
• Provide independent patient care while collaborating with other healthcare team members to meet care goals.
• Serve as a resource for patients, families, and care team members.
