Registered Nurse (RN) Care Manager

CHN is currently assisting a top health plan in the greater Boston area with a need for a RN Care Manager for their SCO plan.




This position is remote based (work form home) and will require visits with members to conduct in person assessments when needed. Travel territory coverage would include: Woburn, Lexington, Medford, and Malden.




Full-time, Monday-Friday (40 hours/week).




No weekends, holidays, or on-call required.




Responsibilities:

-Requires the ability to travel to member's homes, hospitals, and skilled nursing facilities.

-Collaborate with other healthcare professionals to coordinate patient care and ensure effective communication.

-Timely completion of initial and ongoing geriatric assessments

-Completion of the Minimum Data Set-Home Care (MDS-HC)

-Proactive management and follow-up via home visits and telephone according to the member's care plan.
