Summary The Bath VA Medical Center is searching for 1 Registered Nurse (RN) for their Outpatient Patient Aligned Care Team (PACT) Team. This RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Responsibilities Duties include but not limited to; The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
